The Community Connect Initiative is a free service whereby we can provide support, information and referrals to individuals and families who are experiencing significant vulnerabilities and complex needs. This support can be short term or intensive support to those wanting to resolve and improve their current situation. We personalise the support based on our clients’ unique needs, challenges, or situation. We offer support and referral to community members who maybe seeking direction and assistance for a variety of issues. We aim to empower and support our community through information sharing and referrals to reduce barriers and increase their understanding and ability to better cope with challenges.

**The Community Connect Initiative is NOT a counselling program. It is an intensive support program that can be continuing until you are linked with the appropriate agency that will support and meet your needs.**

If you require information and support and wants to know more about the Community Connect Initiative, please do not hesitate to drop into the Neighbourhood Centre or call 4121 2141 to make an appointment. The Community Connect Worker will also be available Wednesdays on a fortnightly basis from 9 am to 12 noon on a ‘first come first serve’, ‘no appointment’ required meeting.

About the Community Connect Initiative

The Community Connect Initiative supports individuals & families who are experiencing challenges & vulnerabilities to connect with the supports that will assist.

This is a FREE program & is for anyone residing in Maryborough & surrounding townships.

The Community Connect Worker (CCW) provides information & referrals for individuals & families who are experiencing challenges, wanting to resolve their needs, are in crisis or want to change their situation.

The Community Connect Worker (CCW) is based at the Maryborough Centre - 25 Ellena Street, Maryborough

The Community Connect Worker can provide:

Information, referral and advocacy
Needs assessment
Connection to community
Domestic and Family Violence support and referral
Case coordination
Crisis support (based on assessment)

Link you with social support
Build your understanding & awareness through providing information
Reduce any barriers stopping you from seeking support
Organise for you to meet service providers & social supports if requested
